About Utah, U.S., Birth Certificates, 1903-1911

In Utah, statewide registration of births and deaths began in 1904, though a few of the recorded births occurred as early as 1903. Birth records are closed by Utah law for 100 years. This collection includes an index and images to birth records for the years 1903–1911. Details include:

  • child's full name
  • sex
  • date of birth
  • place of birth
  • whether a single or multiple births took place (for multiple, number in order of births)
  • whether the parents were married
  • parents’ names, residence, race, ages, birthplaces, occupations, and marital status
  • number of children born to the mother and number living

In addition to birth certificates, you may also find amendments to previously filed birth records, supplemental forms that provided names for children unnamed at the time of the initial registration, and affidavits relating to delayed birth registrations.
